Wedding & Elopement Questionnaires

This is the exact questionnaire I use for my couples. It helps me to plan out specific shots, help them understand what information I need to do my best work, and to avoid awkward situations that lack of planning ahead of time would make. This helps me set myself up for success as well as guide my clients in planning for the day. 

I have also attached a revised edition for elopements to help with the planning of those as well. It includes asking questions about location, permits, etc.
